Care Navigator, Discharge Care Coordinator
Job Description: Care Navigator (Part-Time, Weekends) Remote | Part-time | Friday, Saturday, and Sunday | 8-hour shifts, Pacific time | $30,000 annually About Linea Linea is building a scalable, agentic-led transitions-of-care program for patients who have just left the hospital. For 90 days after discharge, we find and close the gaps that send people back: missed follow-up, medications that were changed but never filled, and social barriers like transportation, food, housing, and cost. We are breaking the mold of traditional transitional care. The usual model puts a nurse or coordinator on the phone with a small caseload, chasing callbacks and faxes, acting as a middleman between the patient and their doctors. It does not scale, and the patient's own voice often gets lost along the way. Automation lets patients speak up. Our automated check-in calls and texts reach every patient on a regular cadence and ask what has changed. Patients raise their needs in their own words: a prescription they cannot afford, an appointment no one scheduled, a symptom that worries them. Our agents turn those needs into tasks, with the patient's words attached, so the gap is clear from the start. We enhance care teams; we do not replace them. The patient's PCP, cardiologist, and nephrologist stay in charge of their care. Our team is the accelerator for expert communication. We take what the patient told us, confirm it against the chart, and get a clear, professional message to the right clinician at the right time. We started in heart failure and chronic kidney disease, including dialysis, and are expanding across the cardio-renal-metabolic space and beyond. We work under shared-risk contracts with ACO partners, so outcomes are what we are measured on. The Role We are looking for a part-time Care Navigator to cover our weekends: every Friday, Saturday, and Sunday, in 8-hour shifts on Pacific time. Weekends matter. Patients are home and easier to reach, and the problems that lead to readmissions do not wait for Monday. You will help patients and their care teams close the gaps that lead to readmissions: missed follow-up, medication changes that never reached the patient, and social barriers that make a care plan hard to follow. When a provider's office is closed, you make sure what you learned is documented and ready for the care team when they reopen. You will work alongside our AI tools and clinical team, reaching patients by phone and text and communicating clearly and professionally with their doctors and care teams. This is a non-clinical role. The patient's own clinicians make every clinical decision. We especially value applicants with medical or nursing training from outside the US, because that background helps you recognize what matters, explain it clearly, and make sure it reaches the right person. Applicants who hold a US nursing or medical license are welcome to apply, but this position does not use or rely on that license. How We Work Technology handles the routine work. Our AI agents take on routine outreach and surface patient needs, so your time goes to the conversations that need a person. The patient's voice comes first. We listen to what patients tell us and make sure their care team hears it. We work with care teams, not around them. You will communicate with physician offices and other providers clearly and professionally. Small team, real ownership. We are an early-stage company, and our navigators help shape how the work gets done. We walk through the details of our process during interviews.
