Case Manager/Social Worker
Advocates for Independence is actively seeking a dedicated Case Manager to join our team, focusing on providing exceptional care to elderly and disabled individuals. This role involves managing a caseload of 25-36 clients residing in community settings. As a full-time position, you'll have the autonomy to set your own work schedule. Most of the work is done from your home Key responsibilities include maintaining monthly contact with each client, conducting comprehensive assessments, coordinating services, advocating for clients' needs, and ensuring timely documentation. You'll collaborate closely with medical teams, adhere to agency and state regulations, and play a vital role in helping clients achieve their wellness and independence goals. To excel in this role, you'll need a strong background in case management or a related healthcare field, with a deep understanding of case management principles, healthcare reimbursement, and psychological care. Effective communication, organization, and problem-solving skills are essential, along with a compassionate approach to teamwork. You must have an active Social Services Worker license. Competitive compensation starting at $25.00 to $28.00 per hour.
