About Valore Health Valore Health is building software that helps community pharmacists work with physicians and other providers to manage chronic conditions such as hypertension and diabetes. The platform supports the pharmacist through each patient visit, care plan and provider review, so pharmacist-led care fits into a normal pharmacy day. We want it designed by people who have actually built and run these programs. About this role This is an advisory and design role. You will not see patients or bill for care on Valore's behalf. You'll partner with our Managing Director, Chief Technology & Product Officer on the clinical workflows, documentation and testing behind the platform. Your job is to make sure it works day to day for patients, pharmacists and providers. Many candidates will keep their current position while working with us part-time. Who we're looking for You've built and implemented a pharmacist-led clinical service or care program and kept it running. That might have been in a community pharmacy, a physician practice, a clinic or a care-management program. You know what it takes to get pharmacists and providers working together, what patients need to stay engaged, and where documentation and software help or get in the way. We're flexible on background and location. Community pharmacy experience is valuable. So is experience building pharmacist-led care programs with physician practices. What you'll do Design the workflows. Map how a chronic care visit really unfolds, from finding the patient through follow-up and handoff to the provider, and help design software that supports each step. Shape the documentation. Help define visit notes, care plans and templates that are complete for the pharmacist and quick for a provider to review. Translate protocols. Help turn collaborative practice agreements and clinical protocols into workflows the software can support. Test and critique. Review prototypes and early releases using test data, and tell us plainly what works and what doesn't. Support rollout. Help shape training, onboarding and quality measures for participating pharmacies and provider partners. Be a clinical sounding board. Join product discussions and occasionally meet with pharmacy and provider partners. What you bring Required PharmD (or BS Pharm with equivalent experience) and an active, unrestricted pharmacist license. 5+ years of pharmacist experience in community pharmacy, ambulatory care, a physician practice, a clinic or a care-management program. Experience building and implementing at least one pharmacist-led clinical service or care program, from design through day-to-day operation. Hands-on chronic disease management experience (hypertension, diabetes or similar). Experience working with physicians or provider practices under a collaborative practice agreement or protocol. Comfort with EHRs and clinical documentation tools, and the ability to explain clinical workflows clearly to engineers and give direct feedback. Preferred Experience helping community pharmacists work with providers to manage chronic conditions. Experience with chronic care management, medication therapy management, transitional care or remote patient monitoring programs. Leadership experience as a pharmacy manager, clinical services lead or program coordinator. Experience with value-based care or quality measures. Experience with clinical informatics, workflow design or rolling out new workflows across multiple sites. Board certification (BCACP or BCPS) or residency training. Why Valore Shape a product from the ground up that could change how pharmacists practice. A flexible, remote schedule that can fit alongside your current role. A clear path to a full-time Clinical Product Lead role, with equity, as the platform scales. A mission-driven team that puts people at the center of a connected care ecosystem.
