About the Job Are you the person others already turn to when they need help? As a Community-Based Case Manager, you support individuals and families in your community by helping them connect to the services they need and stay on track toward their goals. You build trusting relationships and stand beside clients every step of the way.
What You Will Do
- Help clients connect to housing, benefits, employment, education, transportation, and medical care
- Accompany clients to appointments and help them follow through
- Work with clients to set goals and review their progress
- Complete clear, timely progress notes
- Meet regularly with your clinical supervisor and follow program policies
What We Are Looking For
- Caring, reliable, and community-minded
- Comfortable building relationships and communicating with a wide range of people
- Able to complete online training and attend required in-person sessions
- Previous experience is not required
Schedule and Pay
- Flexible schedule that you help set
- Paid for billable hours worked with clients
Training Training is provided online, with some required in-person sessions.
