Functional Psychiatry Provider PMHNP-BC
About Confluence Confluence Functional & Integrative Psychiatry is a boutique mental health practice committed to providing compassionate, evidence-informed, individualized psychiatric care. We recognize that mental health is influenced by more than neurotransmitters and psychiatric diagnoses. Our approach considers the interconnected relationships among neurobiology, nutrition, hormonal health, inflammation, metabolic function, trauma, lifestyle, environmental influences, and emotional well-being. We integrate conventional psychiatric treatment with functional and integrative medicine principles to address symptoms while exploring the underlying factors that may contribute to mental health concerns. Our practice prioritizes meaningful therapeutic relationships, clinical autonomy, collaborative decision-making, and sustainable, high-quality care. We are seeking a thoughtful and experienced psychiatric medication manager who shares our commitment to whole-person mental health and appreciates that successful psychiatric treatment involves more than medication alone. Position Overview The Functional & Integrative Psychiatry Medication Manager provides comprehensive psychiatric assessment, medication management, and individualized treatment planning for an established outpatient telehealth practice. The ideal candidate possesses a strong foundation in conventional psychopharmacology while maintaining intellectual curiosity about the evolving research in nutritional psychiatry, neurobiology, hormonal health, psychoneuroimmunology, and integrative mental health. This role requires clinical independence, sound judgment, excellent communication, and the ability to recognize when a patient's needs exceed the capabilities of a predominantly outpatient telehealth practice. Candidates should be comfortable with a collaborative, relationship-centered model of care rather than high-volume, medication-focused psychiatry. Key ResponsibilitiesPsychiatric Assessment & Medication Management Conduct comprehensive psychiatric evaluations and diagnostic assessments. Provide ongoing medication management for children, adolescents, and adults within the practice's established age range and the clinician's qualifications. Develop individualized, evidence-informed treatment plans incorporating psychiatric, medical, psychosocial, and lifestyle considerations. Prescribe, monitor, and adjust psychotropic medications using current clinical guidelines and best practices. Monitor medication efficacy, tolerability, interactions, adverse effects, and appropriate laboratory parameters. Provide education regarding psychiatric medications, expected benefits, limitations, risks, and alternatives. Assess treatment response and thoughtfully consider medication optimization, reduction, or deprescribing when clinically appropriate. Identify medication-related safety concerns, including polypharmacy, controlled-substance risks, and interactions with supplements or other prescribed treatments. Maintain accurate and timely clinical documentation. Functional & Integrative Psychiatry Incorporate a whole-person perspective into psychiatric assessment and treatment planning. Explore potential contributions from sleep disturbances, nutritional deficiencies, hormonal changes, metabolic dysfunction, chronic illness, and other medically relevant factors. Review pertinent laboratory testing and coordinate additional assessment when clinically indicated. Apply evidence-informed approaches to nutritional psychiatry, lifestyle modification, and behavioral health interventions. Consider the relationship between reproductive hormonal transitions and psychiatric symptoms, including perinatal mental health and perimenopause. Evaluate the appropriateness, safety, and evidence supporting supplements and complementary interventions. Collaborate with primary care, specialty medical clinicians, therapists, and other members of the treatment team. Remain open to emerging research while distinguishing established treatment approaches from experimental or insufficiently validated interventions. Prior formal training in functional or integrative psychiatry is preferred but not required for candidates with a strong clinical foundation and interest in learning. Trauma-Informed & Relationship-Centered Care Deliver compassionate, culturally responsive, trauma-informed psychiatric care. Support shared decision-making and patient autonomy. Understand the influence of trauma, attachment, chronic stress, and interpersonal experiences on psychiatric symptoms. Establish clear, compassionate therapeutic boundaries. Recognize when psychotherapy, specialty evaluation, in-person services, or a higher level of psychiatric care is necessary. Support patients with complex presentations while working within appropriate outpatient clinical and organizational boundaries. Clinical Safety & Professional Responsibility Independently assess suicide risk, self-harm concerns, substance use, psychiatric instability, and other clinical safety considerations. Follow established screening, triage, emergency referral, and escalation protocols. Recognize presentations requiring intensive outpatient, partial hospitalization, residential, inpatient, or emergency psychiatric services. Exercise sound clinical judgment regarding controlled-substance prescribing and complex medication regimens. Coordinate continuity of care and appropriate referrals when patients require services beyond the practice's clinical scope. Comply with state and federal prescribing requirements, professional standards, telehealth regulations, and applicable documentation requirements. Participate in clinical consultation and quality improvement as appropriate.
Required Qualifications
Active, unrestricted license as a Psychiatric Mental Health Nurse Practitioner or appropriately credentialed psychiatric prescribing clinician. Current national board certification where applicable. Prescriptive authority appropriate to the clinician's license and practice location, including DEA registration when required. Minimum of two years of independent or substantially autonomous outpatient psychiatric prescribing experience preferred. Strong knowledge of psychopharmacology, differential diagnosis, psychiatric assessment, and medication safety. Experience managing common outpatient psychiatric conditions, including depression, anxiety, ADHD, PTSD, bipolar disorders, and related presentations. Ability to work independently while collaborating with an interdisciplinary treatment team. Excellent clinical documentation, communication, and time-management skills. Commitment to ethical practice, professional boundaries, and patient-centered care.
Preferred Qualifications
Advanced training or professional experience in functional, integrative, or nutritional psychiatry. Experience in women's mental health, perinatal psychiatry, and reproductive psychiatric conditions. Familiarity with hormonal transitions, thyroid conditions, metabolic health, and their potential relationships with psychiatric symptoms. Experience treating complex trauma and treatment-resistant psychiatric conditions within an appropriate outpatient setting. Training in motivational interviewing, trauma-informed care, or psychotherapy-informed medication management. Interest in psychoneuroimmunology, gut-brain interactions, nutritional science, and lifestyle psychiatry. Familiarity with appropriate laboratory assessment and interdisciplinary medical collaboration. Experience in private practice, telehealth, or a small collaborative clinical setting. Interest in participating in clinical program development and long-term practice growth. Our Practice Environment Confluence offers a clinical environment designed to support thoughtful, individualized psychiatric treatment. Our model emphasizes Quality of care over high patient volume. A functional and integrative perspective grounded in scientific evidence. Transparent communication and clear clinical boundaries. Sustainable workloads and respect for clinicians' professional well-being. Opportunities for continued professional development and growth. Ideal Candidate We are looking for someone who is both clinically confident and intellectually curious. The ideal clinician understands the value of conventional psychiatric medications while recognizing their limitations. They are interested in understanding why a patient is experiencing symptoms, not simply which medication might reduce them. They appreciate the relationships among mind, body, environment, and lived experience without sacrificing clinical rigor or patient safety. Above all, they value ethical practice, compassionate boundaries, collaborative care, and the opportunity to make a meaningful difference in patients' lives.
