Home Health Nurse or LPN | Flexible Hours from PRN to Full-Time

Position: Home Care Registered Nurse (RN) Role & Responsibilities Provide skilled nursing care in patients' homes, including assessments, care planning, medication management, dressing changes, IV therapy, and patient education. Manage post-hospitalization evaluations, recertifications, discharges, and assist with safety and community resource needs. Ensure timely documentation using the Epic system and collaborate with physicians and interdisciplinary teams for continuity of care. Qualifications & Skills Current RN license in Missouri. BLS certification (American Heart Association). Valid driver’s license and auto insurance. Preferred: 1 year of experience in home health or acute care. Key Duties Deliver care across diverse patient populations in a home setting. Assess needs, develop and adjust individualized care plans, and educate patients and families. Monitor patient progress and document services provided. Support patient safety and well-being through various home-based services and instructions. Additional Information Per Diem role with flexible scheduling, including part-time, full-time, and on-call weekends and holidays.
