• Diabetes
• Congestive Heart Failure (CHF)
• Hypertension (HTN)
• End-Stage Renal Disease (ESRD)
• Other chronic and complex medical conditions

DirectMed also specializes in caring for patients receiving outpatient dialysis, providing specialized clinical training and support to our nursing staff. Competitive Per-Visit Compensation We offer the following visit rates:

• $100 – Start of Care (SOC)
• $70 – Regular Skilled Nursing Visit
• $75 – Recertification
• $75 – Resumption of Care
• $75 – Discharge Visit

Additional premium rates may be available for weekend visits. Qualifications & Requirements

• Active Registered Nurse (RN) license in the State of Illinois.
• Bilingual in Polish and English.
• Minimum of one year of nursing experience in a hospital, nursing home, or home health setting.
• Home health and OASIS experience preferred but not required.
• Excellent written and verbal communication skills.
• Ability to work independently and manage assigned patients.
• Reliable transportation.
• Strong organizational and time-management skills.
• A positive, dependable, can-do attitude.
• LPN applicants may also be considered for appropriate nursing opportunities.

Job Responsibilities

• Provide skilled nursing visits to patients in their homes.
• Case manage assigned patients and coordinate their care with physicians and other healthcare professionals.
• Conduct comprehensive nursing assessments.
• Complete Start of Care, Recertification, Resumption of Care, and Discharge assessments as assigned.
• Develop and update individualized patient plans of care.
• Provide medication management, wound care, chronic disease education, and other skilled nursing services.
• Communicate with patients and families in Polish when appropriate.
• Coordinate visit schedules directly with assigned patients.
• Complete all visit documentation within 48 business hours of the home visit.
• Maintain professional communication with DirectMed's clinical and administrative teams.
