In Home Healthcare LVN - Full Time Days or Nights

We are seeking a compassionate Licensed Vocational Nurse (LVN) to provide skilled nursing care in a home setting for pediatric patients with complex needs, including feeding tube management and medication administration.

Key Responsibilities

Include Delivering and documenting patient care according to physician orders and care plans Assessing patient condition, recognizing changes, and reporting to supervisors or physicians Educating patients and families on disease management, self-care, and prevention Maintaining confidentiality, professional boundaries, and compliance with regulations Participating in ongoing education and quality improvement activities

Qualifications

Active, unencumbered LPN/LVN license in the practicing state Current CPR certification with hands-on component TB test within the past year Reliable transportation and ability to travel Six months of clinical nursing experience preferred; home health experience desirable Skills & Abilities Attention to detail, organization, and effective communication Problem-solving and conflict resolution skills Ability to lift up to 50 pounds and perform physical patient care tasks Proficiency in assessment, documentation, and adherence to policies Work environment: Home-based care with exposure to varied conditions, including potential contact with blood and bodily fluids. Must respond effectively to emergencies and maintain safety standards.
