This is a hybrid position (2 days remote, 3 days in the field), giving you the flexibility to work from home while building strong, in-person connections with members across Los Angeles County.

Key Responsibilities

Serve as the primary point of contact for members and coordinate their care Develop and manage personalized care plans with members and providers Collaborate with healthcare teams and community resources Support members through education, advocacy, and engagement Identify and address gaps in care Maintain accurate, timely documentation
