Benefits: 401(k) Flexible schedule About the Role: Karing Hearts Home Health is looking for a compassionate and skilled Per Diem Registered Nurse to join our dedicated home health team in Ventura, CA. Responsibilities: Conduct skilled nursing assessments and develop individualized patient care plans Administer medications, wound care, IV therapy, and other clinical treatments in the home setting Monitor and evaluate patient health status, reporting changes to the physician as needed Educate patients and family members on disease management, medications, and home safety Coordinate care with interdisciplinary team members including therapists, aides, and physicians Accurately document patient visits, progress notes, and care plans in compliance with Medicare/Medicaid guidelines Ensure compliance with all state, federal, and agency home health regulations Requirements: Active California Registered Nurse (RN) license in good standing Minimum 1 year of clinical nursing experience; home health experience preferred Current BLS/CPR certification Valid California driver's license and reliable transportation Familiarity with OASIS documentation and Medicare home health guidelines is a plus Strong communication, time management, and critical thinking skills Compassionate, self-motivated, and comfortable working independently in the field About Us: Karing Hearts Home Health is a trusted home health provider serving the Ventura, CA community with a heartfelt commitment to exceptional patient care.
