Remote House Calls Nurse Practitioner

Essen Health Care is seeking a Remote House Calls Nurse Practitioner to support our in-home medical services program. This role provides virtual clinical oversight, patient assessments, care coordination, and provider support for patients receiving care in their homes. Responsibilities

• Conduct telehealth visits for patients enrolled in the House Calls program
• Manage chronic conditions such as diabetes, hypertension, CHF, COPD, and geriatric syndromes if the patient requires primary care services
• Perform care management needs assessments, follow-ups, and care plan adjustments
• Review patient histories, labs, imaging, and diagnostic results
• Support medication management and reconciliation
• Coordinate care with primary care providers (if patients already have one), specialists, and care management teams
• Document visits and clinical decisions accurately in the EMR
• Ensure compliance with clinical protocols, regulatory requirements, and participate in interdisciplinary case reviews and quality improvement initiatives

Qualifications

• Active Nurse Practitioner (NP) license in the required state(s)
• National NP certification (ANCC or AANP)
• Minimum 2+ years of clinical experience as a Nurse Practitioner
• Experience in house calls, home health, geriatrics, primary care, or internal medicine strongly preferred
• Prior telehealth or remote care experience preferred
• Strong clinical judgment and comfort managing complex patients remotely
• Excellent communication and documentation skills
• Must be licensed in NY and one of the other states listed or willing to obtain
• Proficiency with EMR/EHR systems, telehealth platforms, and remote monitoring tools
• Active DEA registration (or willingness to obtain, if applicable)
