Senior Practice Transformation Specialist
The CMA-PSO is looking for a Senior Practice Transformation Specialist to serve as a strategic advisor, operational coach, resource developer, and implementation partner to independent physician practices, IPAs, and Federally Qualified Health Centers participating in multiple large-scale Medi-Cal quality improvement, encounter data, population health, and value-based care initiatives. The position independently manages concurrent projects, provider relationships, deliverables, timelines, and stakeholder needs; analyzes quality and operational data; and translates project goals and subject-matter expertise into practical implementation strategies, polished resources, and adult-learning experiences. Success requires strong judgment, facilitation, project management, data-informed problem solving, and consistent delivery of measurable improvements and reusable, field-tested practice transformation resources. Within the first six months, success means building trusted relationships, leading effective coaching and learning sessions, delivering polished resources, and moving multiple projects toward measurable quality and operational goals. The strongest candidates bring sound judgment, strategic and analytical thinking, adult-learning and facilitation skills, and the ability to simplify complex information for clinical, operational, and executive audiences.
