Transplant Nurse II
About This Role The Transplant Nurse II is responsible for providing case and medical management for members receiving transplant services, focusing on optimizing member healthcare across the care continuum. This role is ideal for nurses looking to advance their skills in a supportive and flexible environment. Highlights Pay: $75,680 - $119,196/year Location: Latham, NY Schedule: Full-time, Monday - Friday, 8:00am - 4:30pm EST What You'll Do Conduct assessments to identify individual needs and develop care management plans. Implement care plans by facilitating authorizations/referrals as appropriate. Coordinate internal and external resources to meet identified needs. Monitor and evaluate the effectiveness of care management plans and modify as needed. Interface with Medical Directors and Physician Advisors on care management treatment plans. Assist in problem-solving with providers, claims, or service issues. Collaborate with healthcare providers and members to promote quality outcomes. Ensure medically appropriate, high-quality, cost-effective care through assessments. Conduct pre-certification and appropriateness of treatment setting reviews. Facilitate member care transitions through the healthcare continuum.
Requirements
Education: AS in nursing required; BA/BS or higher in a health-related field preferred. Experience: Minimum of 3 years of clinical experience required. Licenses & Certifications: Current unrestricted RN license in applicable state(s) required. Certification as a care manager preferred.
