Our primary care services are provided by a team of licensed clinicians in the comfort of the patient’s home. By bringing together physicians, nurse practitioners, social workers, paramedics, and other healthcare professionals, WellBe delivers an extra level of support to patients in their communities. The high-risk population we serve is often underserved and faces significant challenges accessing care. To address these barriers, we bring care to the patient rather than requiring the patient to come to us. WellBe’s clinician-led geriatric care teams provide concierge-level medical care and social support in the home while coordinating care across the entire continuum—from chronic and urgent care at home to hospitals, skilled nursing facilities, assisted living, palliative care, and end-of-life care.
