Medical Assistant - Chronic Care Management / Remote Pt Monitoring
Compensation: $22–$27 per hour, depending on experience Employment Type: Full-Time Desert Bloom Family Medicine is seeking a highly motivated, experienced Medical Assistant (MA) to join our growing Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) program. We are specifically looking for a candidate with extensive, hands-on experience in CCM and RPM. Only candidates with demonstrated CCM/RPM experience will be considered. This is NOT an entry-level position.
Key Responsibilities
- Conduct monthly outbound CCM and RPM calls to enrolled patients.
- Monitor patient-reported symptoms, chronic conditions, and remote device readings.
- Document patient interactions, care coordination activities, and time spent on CCM/RPM services accurately in the EHR.
- Assist with patient enrollment, education, and ongoing engagement.
- Coordinate care with providers and clinical staff, escalating clinical concerns appropriately.
- Help ensure monthly documentation and billing requirements are met.
- Assist with maintaining and growing a successful CCM/RPM program.
Qualifications
- Medical Assistant certification or diploma preferred.
- Prior direct experience in CCM and RPM is required.
- Strong understanding of Medicare CCM/RPM documentation and workflows.
- Experience conducting patient outreach and managing chronic disease populations.
- Excellent communication, organizational, and computer skills.
- Ability to work independently and manage a large patient panel.
- Bilingual English/Spanish is a plus.
