Patient Outreach & Enrollment Manager
About Companian Companian works with healthcare practices to help patients manage their health between doctor visits. Our care team supports patients with appointments, referrals, medical records, medication coordination, and access to community resources. We help eligible patients access care management services, including Chronic Care Management (CCM) and Principal Care Management (PCM), subject to Medicare’s eligibility and coverage requirements. We’re growing across Florida and the central U.S. and building the team that helps patients understand and access this support. The role We’re hiring a Patient Outreach & Enrollment Manager to build and lead the function that connects patients at our partner practices with care management. You’ll own enrollment end to end: the outreach strategy, the call team, the scripts and consent workflow, and the numbers. Early on, you’ll be hands-on, making calls yourself to set the standard and refine our approach. As we scale, you’ll hire, train, and manage a team of outreach representatives and make sure every patient conversation is accurate, respectful, and compliant. What you’ll do Build and lead the team Recruit, onboard, and train outreach and enrollment representatives as we expand into new practices and regions. Set daily and weekly targets, run regular coaching sessions, and manage performance with clear, fair expectations. Listen to calls and run quality reviews, giving specific feedback on accuracy, tone, and patient experience. Create a culture where a patient’s “no” is respected and quality matters as much as volume. Own the outreach operation Design outreach cadences across phone, voicemail, and text where permitted, within calling hours, contact limits, and opt-out requirements. Manage patient lists with partner practices: intake, prioritization, follow-up queues, and do-not-call handling. Maintain scripts, objection-handling guides, and approved materials, working with clinical leadership to keep them accurate and compliant. Oversee the consent process and documentation, ensuring required disclosures are delivered and recorded and that enrollment information reaches the patient’s medical record. Coordinate with the clinical team on eligibility checks, initiating visits, and handoff to care advocates. Drive results Own enrollment metrics: calls, live conversations, conversion, consent accuracy, and time to first care contact. Build simple reporting for leadership and partner practices, spot what’s working, and fix what isn’t. Bring patient and practice feedback back into our scripts, materials, and onboarding. Help launch outreach for each new partner practice, from list setup to first enrollments. Stay hands-on Carry your own call volume in the early stage and step in on escalations and difficult conversations. What you’ll bring 3+ years in healthcare outreach, patient enrollment, or care coordination, ideally in CCM, PCM, RPM, or other Medicare care management programs. 1+ year leading, training, or coaching an outbound phone team. A track record of hitting enrollment or conversion targets without cutting corners on patient experience. Working knowledge of Medicare care management consent requirements, HIPAA, and outbound calling rules (TCPA, opt-out handling). Comfort with data: you track the numbers, read a dashboard, and turn it into decisions. Experience with a CRM or call platform; HubSpot a plus. Clear, warm communication with older adults and caregivers. Startup energy: comfortable building processes from scratch and doing the work yourself before handing it off. Bilingual English/Spanish a plus.
