Physical Therapist (Home Health)

Location: Brookings, OR Employment Type: Full-Time | Onsite / Community-Based Schedule: Monday-Friday, 8:00 AM-5:00 PM Setting: Home Health / Community-Based Patient Care (Patients' Homes) Job Overview We are seeking a compassionate Physical Therapist to provide home-based rehabilitation services, helping patients improve mobility, restore function, and maintain independence.

Key Responsibilities

Evaluate patients and develop individualized treatment plans. Provide therapeutic exercises, gait training, mobility training, and functional rehabilitation. Educate patients and caregivers on home exercise programs and safe mobility. Monitor patient progress and maintain accurate clinical documentation. Coordinate care with physicians and interdisciplinary healthcare teams. Supervise Physical Therapist Assistants as required.

Requirements

Graduate of an accredited Physical Therapy program. Active, unrestricted Oregon Physical Therapist license. Current BLS certification or ability to obtain before starting. Valid driver's license, auto insurance, and reliable transportation. Strong communication, organizational, and clinical assessment skills. One year of experience preferred; new graduates are welcome.
