PRN Nurse Practitioner — Remote Medicare Chronic Care (All States)
$75 per hour — Florida, Georgia, New York, Texas, Washington. $90 per hour — Arizona, Colorado, Massachusetts, Nevada, New Mexico, Oregon. 1099 independent contractor. Fully remote. Telephonic, not video. You must hold an active NP/APRN licence in at least one of the states above. TeleMD Health works with clinician-owned practices delivering Medicare chronic care management and remote patient monitoring. We are scheduling PRN hours now. PRN means as-needed — you tell us the days and hours you want and we schedule you into them. Blocks of two to four hours are fine. There is a daily sign-up if you want to pick up an extra day. THE WORK Telephonic chronic care management for Medicare patients. You conduct the initiating visit and establish the care plan, then monthly check-in calls to manage the patient's chronic conditions and update the plan. Where a patient has a connected device, you review their readings. WE TRAIN YOU Before you see patients we train you on chronic care management, remote patient monitoring and our platform. Nobody starts cold. WHAT YOU NEED Active, unencumbered NP/APRN licence in one or more of the states listed above. Your own professional liability insurance — this is PRN contract work, so you carry your own cover. NSO and CM&F both write self-employed part-time policies. Current NPI. Reliable internet and a private workspace. Medicare enrolment through PECOS is a plus, not a requirement — if you are licensed but not enrolled, our credentialing team files it for you at no cost.
