Regional Sales Director, Provider Market
Regional Sales Director, Provider Market, North Central Florida Reports To: Chief Commercial Officer Location: Remote, based in the Ocala to Lake City corridor Territory: North Central Florida: Ocala north to Lake City, including Gainesville and The Villages Travel: 50 to 70% within territory Employment Type: Full time, exempt Anticipated Start: January 2027 Base Salary: $180,000 OTE (2027): $350,000 About GroundGame Health GroundGame Health exists because the healthcare system was never designed to meet people where they actually live. Every day, millions of Americans struggle to manage their health not because they lack a diagnosis or a prescription, but because they cannot get to the pharmacy, do not have stable housing, cannot afford groceries that support their treatment plan, or have no one to help them navigate a system that was built without them in mind. We are changing that. GGH is a tech enabled social care management organization that deploys trained, certified community health workers (CHWs) under physician supervision to deliver Community Health Integration (CHI) and Enhanced Care Management (ECM) services for Medicare and Medicaid patients. Our proprietary platform, Implify, powers documentation, billing, and care coordination so that providers can offer whole person care without building the infrastructure themselves. A peer reviewed study published in NEJM Catalyst demonstrated statistically significant cost reductions in populations served by programs GGH powers. This is not theoretical. It is proven, published, and reimbursable. We are building a national provider facing sales team to bring this model directly to FQHCs, independent practices, and health systems. If you believe healthcare should be rebuilt from the community up, and you want to sell something that genuinely changes lives, we want to talk to you. The Opportunity This is a ground floor sales role in a company executing a strategic pivot from payer facing to provider facing revenue. You will be one of the first regional sales directors hired to build the provider model pipeline nationally. The product you are selling is straightforward: GGH provides the CHW workforce, supervision infrastructure, technology platform, and revenue cycle management so that a provider group can bill Medicare and Medicaid for CHW services without hiring, training, or managing the program themselves. The provider keeps a margin on every member served. The CHW work is reimbursable under established CMS billing codes (G0019/G0022) that went live in January 2024, and a growing number of state Medicaid programs are adding parallel reimbursement. The hardest part of this sale is getting 15 minutes on a medical director's calendar. Once you are in the room, you lead with what matters most: patients with uncontrolled diabetes who cannot get to the pharmacy, post discharge patients who miss follow ups because they do not have stable housing, members cycling through the ED because no one has ever helped them navigate the system. Community health workers change those outcomes, and the clinical evidence is published and peer reviewed. When the provider sees that this is real clinical value for their most complex patients, and that their practice can generate new monthly revenue from services those patients already need, the conversation shifts from "why" to "when." Territory Overview Your territory is the corridor running roughly from Ocala north to Lake City, including Gainesville and The Villages. This area was chosen deliberately. It has the population base and the payer mix profile that fits the GGH provider model: a high share of Medicare fee for service and a low share of Medicare Advantage. South Florida (Miami and Fort Lauderdale) and Tampa are outside this territory. That payer mix matters because Medicare fee for service is where the CMS CHI codes (G0019/G0022, live since January 2024) work most directly. A practice bills Medicare for the CHW work under its own NPI, GGH supplies the CHWs, supervision infrastructure, Implify platform, and revenue cycle support, and the practice keeps a margin on every member served. Closing provider deals takes face to face follow up, which is why this role is a field role built for someone who lives in the corridor and can be in clinics, not on video calls, across the territory.
What You Will Do
Build and own the provider sales pipeline across your territory, targeting FQHCs, independent primary care practices, community health centers, and small health systems with high Medicaid and Medicare patient volumes. Conduct outreach and set meetings through a combination of cold calling, email campaigns, conference networking, lunch and learn events, and referral development. Your primary metric in Year 1 is qualified meetings per week. Deliver the GGH value proposition to practice administrators, medical directors, CMOs, and C suite decision makers, clearly articulating the CHI/ECM revenue opportunity, the Implify platform, and the GGH operational model. Manage the full sales cycle from initial outreach through needs analysis, proposal, contracting, and handoff to operations. You will use a defined stage gate pipeline methodology and report progress weekly to the CCO. Develop deep market intelligence on your territory including provider networks, health plan relationships, payer reimbursement policies, and competitive landscape. You will be the company expert on your region. Collaborate with marketing and operations to refine collateral, tailor proposals, and ensure smooth implementation handoffs. Provide field feedback to improve the pitch, the product, and the process. Represent GGH at industry conferences, state primary care association meetings, FQHC consortium events, and community health convenings within your territory. Maintain disciplined CRM hygiene with accurate pipeline data, stage progression, and forecasting. Your pipeline report feeds directly into the company's investment banking and board reporting.
Required Qualifications
Proven experience selling complex tech enabled solutions into provider organizations such as FQHCs, independent practices, physician groups, and small health systems. Experience with IDNs or academic medical centers is welcome but not required. B2B sales experience in healthcare services, health IT, remote patient monitoring, chronic care management, population health, care management, or a related field. You have sold to provider organizations (FQHCs, physician groups, clinics, or health systems) and understand how practice level decisions get made. Experience selling a service that practices bill payers for, such as RPM or CCM, is especially valuable because you already know how to explain reimbursement to a practice. Demonstrated ability to open doors. You have a track record of booking meetings with hard to reach decision makers through persistence, creativity, and professionalism. References should confirm you can generate pipeline from scratch, not just manage inbound leads. Willingness to travel 50 to 70% within your territory. This is a field sales role. You will be in clinics, at board meetings, at state association conferences, and in provider break rooms. Weeks with 3 to 4 days of travel are normal. You must physically live within the Ocala to Lake City corridor or be willing to relocate there before your start date. Comfort with early stage environments. This is not a role where you inherit a book of business. You will be building playbooks, not following them. You must be energized, not frustrated, by ambiguity and iteration. Bachelor's degree. Business, healthcare administration, public health, or a related field preferred. An MPH, MHA, or MBA is a plus but not required if you have strong relevant experience.
Preferred Qualifications
Existing relationships with provider groups, primary care associations, or health plan network teams in the assigned territory are a strong plus, but not required if you have a proven record of opening new doors. Working knowledge of Medicare fee for service, Medicaid, or value based care. You do not need to be a billing expert, but you must be conversant in how provider reimbursement works and comfortable discussing fee schedules, Superbill to EOB payment cycles, and payer provider dynamics. Familiarity with the North Florida provider landscape is a strong plus. Familiarity with community health worker programs, social determinants of health interventions, or care management services. Experience in a startup or high growth company where you built pipeline and process simultaneously.
Compensation and Benefits
GGH is hiring for a role where the product sells itself once you get the meeting. The compensation structure reflects that reality: a competitive base salary that respects the ramp period, with accelerating commissions that reward pipeline generation and closed deals. Base Salary: $180,000. The base is designed to be competitive and livable from day one while you build your pipeline. Commission. The revenue based commission plan is payable from your first day. Revenue above quota pays at 1.5x commission rate. Net new provider logos carry a one time bonus. 2027 OTE: $350,000. Compensation is driven by bookings and revenue. A top performer with a full pipeline should expect to exceed this range. Health, dental, and vision insurance. Company sponsored 401k. Travel expense reimbursement. Unlimited PTO policy.
