Their leadership brings deep roots in health AI research, and they’re scaling quickly. What You’ll Do Conduct onboarding calls with new patients and their families Build individualized care plans covering symptoms, modifiable risk factors, and medical and non-medical interventions Lead monthly check-ins: assess changes from baseline, recommend care resources, and coordinate with caregivers and clinics Manage care logistics, including referral scheduling, medication refill flags, and appointment reminders Find and vet regional and disease-specific resources (exercise, nutrition, sleep, social engagement) for your patients Partner with the technology team, giving feedback on the care platform and helping shape how AI fits into patient care What You Bring Active RN license (required) CCM/PCM experience preferred Strong clinical knowledge: you can comfortably explain chronic conditions, how they progress, what families should watch for, and what helps A scrappy, adaptable approach: comfortable with new tools, untested ideas, and building without a playbook Patient-first mindset: you see this work as a service, not just a job Honest and low-ego: you own mistakes and work through challenges with grit Hunger to grow into more responsibility over time Why Join Significant ownership and autonomy over the clinical program Substantial growth in scope and compensation as the company scales A mission-driven team fully committed to each other and to exceptional patient care
